Some say that if you make a mistake in a wazeefa then the opposite effect will occur and you will suffer from side effects that is why you must be accurate or you must get ijaazat from someone qualified.
can someone shed more light on this issue
has anyone experienced or knows anyone who has made these errors

I have not come across what you are across, but in any case extreme care and attention should be given at all times during any time of prayer so as to avoid making errors.

Without getting into any technical discussions, yes it is true - that one should consult with someone more knowledgeable in the field before venturing into anything. I know someone who has mental instability - and the only credible reason is because he was reading wazeefas way above his head. The wisdoms in our practical lives should also apply to our spiritual lives. For example we won't attempt going sky-diving without proper knowledge and guidance. The same applies in spiritual matters - we should always always seek counselling with our murshid.
Another important point, is that once we take it upon ourselves to recite any certain wazeefa - we should ensure that we remain steadfast in it... and unless instructed by our Murshid - we should not take on any other wazeefas and leave the previous ones. One cannot fly if they haven't yet learnt to walk - and when one is flying - he must still know how to walk...

I mean what is the reason for this 'mental instability' and how long has this person been in this condition are factors .... Allah (Subhanahu wa Ta'ala) knows best.
